Daily Digital Health Check Attestations


Dear families,


The Edmonds School District is using a digital daily health check attestation system as part of our mitigation practices to keep our students, staff and families safe during the COVID-19 pandemic.


What is a digital daily health check attestation form? 

It is a way for families to provide proof (the completed digital form) their student is healthy to attend school -or- that their student needs to stay home due to symptoms or exposure related to COVID-19. 


Who needs to fill out the daily health check attestation form?

The district is asking all families to fill out a form for each student, each school day, beginning Monday, March 22. The health check attestation form will only be sent to Guardian One of Family One in Skyward Family Access. Please make sure you have the correct email address listed in Skyward Family Access for the parent or guardian that should receive the daily email.


My student is attending school just two days a week in the hybrid model, do we still need to fill it out every day?
Yes. Each student should have a daily health check attestation form filled out every school day, even if remote. That means families fill out the digital form for their students five days a week. The health check attestation form will be emailed each morning around 5 a.m. to Guardian One of Family One in Skyward Family Access.


Why do we need to fill this out on days a student is not on campus, or in the case that my student remains in fully remote learning for the rest of the school year? 

We want students and families to get familiar with this daily process. By filling out this form daily, it creates a routine of behavior. It also ensures that students who may be remote will have the required health information filled out before participating in any on-site activities or services. 


Which parent/guardian will receive the daily health check attestation form in their email?

The health check attestation form will only be sent to Guardian One of Family One in Skyward Family Access. Please make sure you have the correct email address listed in Skyward Family Access for the parent or guardian who you would like to receive the daily email. 


Can 7th-12th grade students fill this form out themselves?

Yes. Per Snohomish Health District guidelines, families with students in 7th through 12th grade can choose to give their student permission to complete the daily health check attestation form themselves. In order to do this, families must opt-in to receive the daily form via text message. As you are filling out the form, you will see a section where you can enter your student’s cell phone number. Once you do that, the student will get a daily text and the parent will get a daily email. Whoever fills out the form first, that is the information the district will collect for that day. 


What about students who live with different parents at different times? How can both parents/guardians receive the daily email?

Guardian One of Family One in Skyward Family Access will receive the daily email. For a second household, we suggest you follow the steps to have the daily text message sent to a second family, if applicable. This would mean both households would receive the daily health check attestation form, one via email, the other via text. Whoever fills out the form first, that is the information the district will collect for that day. 


What about childcare before school, after school or all day care? 

The district is in partnership with Right At School, Kids Krew and the YMCA to provide before, after and all day childcare services (please see this webpage for more childcare information)


Families enrolled in these programs will fill out the same daily health check attestation form for their childcare provider as the district is using for all students and staff. Families participating in childcare need to fill out the daily form each morning for before school childcare. If you are only participating in after school care, the form you fill out for your student to attend school that day will be shared with your childcare provider. The goal is for families to only need to fill it out once a day.
